Listed below are the forms that need to be completed for a Change of Corporate Structure: 

        1. Limited Liability Company and Corporate Report of Changes - DR8177 
        2. Individual History Record - DR8404-1 

Fees due

    Payable to City and County of Broomfield
    Change of Corporate Structure:         $100.00 

    Payable to:  Colorado Bureau of Investigation (CBI):
    (for anyone new on the license with 10% or more interest) 
    Fingerprinting fee                             $ 38.50 each person 

Those listed on the application as officers and stockholders of 10% or greater need to complete the Individual History Record forms and need to be fingerprinted by the Police Department for a background check. A copy of the minutes of the meeting appointing new officers needs to be included in the packet. The Change of Corporate Structure Packet must be submitted in duplicate. All documents must be printed single-sided on standard 8.5x11 white paper. Fingerprinting is done at the Broomfield Police Department on Monday and Tuesday from 9:00 a.m. to 4:00 p.m. and on Monday evening from 6 p.m. to 8 p.m. The fingerprinting fee payable to CBI is $38.50 and should be submitted to the Clerk’s office, along with the fingerprint card.
